How to start a claim
Print out the PDF document and sign on page three, remember to provide complete information on your company and include your business card with the agreement.
Provide substation of the debt, copies (do NOT send originals) of memo, invoice and or statement. Include ALL known address, telephone, cell, fax and email addresses of debtor and debtor companies.
Include if any address is known as a bad address or bad telephone number (if address is know as bad address include so a starting point may be obtained for skip trace services) If known provide personal home addresses and telephone numbers of both debtors or debtor relatives and or associates. Send the entire information package with signed and dated agreement to:
